Starting a new medication or treatment often comes with a lot of questions. Upon enrollment, a Patient Access Liaison is assigned to your patient for dedicated one-on-one support and your patient’s non-medical, logistical needs—from treatment initiation to management and beyond. Even better, they have a team of support behind them working to help your patient too. The Patient Access Liaison is a part of the Horizon By Your Side team and is committed to helping your patient every step of the way.

Once your patient is enrolled in Horizon By Your Side, they will receive a welcome call from a Patient Access Liaison within one business day. After that, your patient can call, text, or email them when they need logistical support.
The Regional Access Liaison has the expertise and tools to support the patient by educating on patient benefits, options for site of care, prior authorization requirements, payer policies and coding and claim submissions. The Regional Access Liaison also can provide you with updated information about your patient's access journey.
For help finding a site of care for your patient, you can contact us for site of care support. Our Site of Care Team Members establish business-to-business relationships with sites of care, expand infusion center options, provide coding and billing information, and offer infusion and payer access education. They can provide site of care options and infusion center access for your patients.
For patients on ACTIMMUNE® (Interferon gamma-1b), the Clinical Nurse Educator is part of a dedicated Horizon By Your Side team that is ready to help your patient, from answering questions as they start treatment through every step along the way. The Clinical Nurse Educator is available to provide information and education to your patient and their family and can meet in person or by email, text, or phone.

For patients on RAVICTI® (glycerol phenylbutyrate), PROCYSBI® (cysteamine bitartrate), or ACTIMMUNE® (Interferon gamma-1b), the Horizon By Your Side team includes Case Managers who can provide ongoing support and education for your patients and their families. The Case Manager assigned to your patient may also be in touch with your office to make sure important insurance information is properly shared. These comprehensive services are free of charge for your patients.

Through our team’s specialized knowledge of how insurance companies work and understanding specific benefits, the Patient Access Liaison or Clinical Nurse Educator can help your patient understand the insurance process by walking them through coverage and possible financial assistance options.
As an industry partner, we have limitations that we must respect. We cannot guarantee access or reimbursement for our medicines; however, we can educate you and your staff about gaining access to the medicine and various patient financial support programs. Similarly, we cannot provide medical advice to your patient about the medicines you prescribe or act as an extension of—or substitution for—your staff.
With any newly approved medicine, the time from prescribing the medicine to treatment may take longer than initially expected, as payers learn about the medicine and develop their policies. We are committed to working with payers to enhance access and learning more about payer policies so we can educate you about these nuances. Together, we can advocate for your patient to gain access to the medicines you believe will improve their lives.

Patient enrollment in Horizon By Your Side is easy. Download, complete, and submit the Patient Enrollment Form below. There is a specific section on the form required in order to initiate enrollment for your patient in Horizon By Your Side. Have your healthcare team work with your patient to finalize the form.
Once your patient is enrolled, you will receive a comprehensive summary of your patient’s benefits from the Horizon By Your Side team. We will be there to provide support when they need it throughout their treatment experience.
