Patient Access Liaison

Starting a new medicine or treatment often comes with a lot of questions. Once enrolled, you will be paired with a Patient Access Liaison (PAL). Your PAL provides you with dedicated, one-on-one support. They work directly with you to answer non-medical, logistical questions and provide support once you have been prescribed a Horizon medicine and you have completed enrollment.

Clinical Nurse Educator

If you’re a patient on ACTIMMUNE® (Interferon gamma-1b), when you begin your therapy, you are assigned a Clinical Nurse Educator (CNE). Your CNE provides you with dedicated, one-on-one support. They work directly with you to answer non-medical, logistical questions and provide support once you have been prescribed ACTIMMUNE® and you have completed enrollment.

In addition, your Clinical Nurse Educator can help you:

  • Get non-medical educational support
  • Avoid surprises at the start of treatment by talking about what to expect
  • Discuss how your new treatment can fit into your routine and meet your goals
  • Understand cost assistance options
  • Learn about the insurance process
  • Coordinate delivery with the specialty pharmacy to schedule shipments of your medicine
  • Get injection training and support
  • Connect to a sharps disposal program for easy needle and syringe pickup and disposal
  • Receive refill reminders to help you stay on ACTIMMUNE® as your doctor has instructed
  • Connect to advocacy groups for more support and inform you of events in your area

  • Do you have commercial insurance, like a preferred provider organization (PPO) or health maintenance organization (HMO)? If so, you may qualify for a lower co-pay*
  • Do you have government insurance, such as Medicare, Medicare Part D, Medicaid, Medigap, VA, CHAMPUS, Department of Defense, TRICARE, or any state, patient foundation, or other pharmaceutical program? If so, there may be additional support available for you 
  • Once you and your healthcare team agree on a plan that works best for you, your Patient Access Liaison or Clinical Nurse Educator will talk to you about the insurance approval process and what your potential out-of-pocket treatment costs may be

Once you and your healthcare team decide that a Horizon treatment is right for you, enrolling in Horizon By Your Side is easy. You can join in one of two ways:

  • Patient Access Liaisons and Clinical Nurse Educators can help you sign up and answer non-medical or logistical questions

Option 2: Sign up through your doctor’s office. They will have the enrollment form and can submit it for you after you’ve signed it

  • Once you’ve signed the Patient Enrollment Form, your Patient Access Liaison or Clinical Nurse Educator will take the first step by calling you to introduce themselves and help you with non-medical or logistical questions

*The Horizon Commercial Co-Pay Program may be available to patients who meet the following minimum criteria:

  • Patient’s prescription cannot be paid in part or in full by any government-funded program including but not limited to: Medicare, Medicare Part D, Medicaid, Medigap, VA, CHAMPUS, Department of Defense (DOD), TRICARE, or any state, patient foundation, or other pharmaceutical program
  • Patient is prescribed a covered Horizon rare disease medication for an indication approved by the Food and Drug Administration; the indication for each product is shown in its prescribing information
  • Patient is a resident of the United States.
  • Patient must be commercially insured and have financial responsibility for a portion of the drug and/or infusion cost if applicable

The assistance offered under this co-pay program is subject to additional terms and conditions, including but not limited to the following:
