Horizon By Your Side is a patient support program with dedicated team members who take a personalized approach to meeting the unique needs of patients prescribed Horizon medicines. We will be by your side. It can make all the difference to have someone in your corner: a dedicated team member who will support you throughout your treatment. The Horizon By Your Side team is committed to improving lives with services and support for you and your family.
Together, we will:
CONNECT: You will be paired with a Patient Access Liaison or Clinical Nurse Educator who will be there to support you throughout treatment when you need it
COORDINATE: The Horizon By Your Side team will work with you and your healthcare team to make sure logistical, non-medical efforts are coordinated properly
CHAMPION: Your Patient Access Liaison or Clinical Nurse Educator will be your point of contact and help you to build confidence throughout your treatment
Starting a new medication or treatment often comes with a lot of questions. Upon enrollment, a Patient Access Liaison is assigned to you for your dedicated, one-on-one support and logistical needs. Even better, they have a team of support behind them working to help you too. Your Patient Access Liaison and the whole Horizon By Your Side team are committed to helping you understand your treatment experience every step of the way.
